First generation batteries were too big and overheated. Replacement generation batteries were from a different manufacturer, but the rush to get them out caused Samsung to miss a defect that caused the explosions of the second generation.
Samsung says they're definitely going to make octuple-sure that this sort of thing doesn't ever happen again.
