I generally build and test on medium.  it seems to work for difficulty.  some missions easy, other missions hard, and some, almost impossible damn lucky to finish and leave that mission behind.

The middle option.  Not too easy, not too hard.  There's nothing worse than having to replay a mission twenty times until everything goes right.

 
I like campaigns hard/insane now that I have started playing multi...

I'd like to see one that's balanced well for medium difficulty at Medium difficulty, and that gets significantly harder (and adds bonus features/missions) on hard/insane.
