While the pathogen exists, it's mutating. The only thing treatment does as far as that goes is isolate the strains that are immune, it does not MAKE anything immune.

evolution - selection == mutation
evolution != mutation
Oh, Jesus! you are wrong! you have the evolutionary understanding of Fred Phelps! without being exposed to the treatment those microbes that have some small mutation that could lead to resistance will do no better, and possibly worse, than the quadrillion that do not have the mutation. evolution REQUIRES a selection pressure in order to work. if what you are saying was true no treatment would ever be effective because all bacteria in the universe would have evolved resistance to every treatment that could be invented billions of years before the treatments were developed.
selection is what allows evolution to move in a particular direction, without selection mutation just jitters about randomly
