I recently got a friend interested in trying out FSO, and we're having issues. She installed FS2 from GOG and ran the FSO installer, and now we can't get the game to actually launch. When she tries, it complains that "FS Open couldn't generate a flag file", and fails with status 255. I attempted to have her generate an fs2_open.log file, but the "Advanced Settings" tab is blank aside from the same complaint about a missing flag file. We followed the rest of the steps anyway, but there was still no log file.

At this point, I'm out of ideas. She's running OSX (version 10.9.5). I'd call myself technically adept (though fairly unfamiliar with OSX), but my friend isn't, and she's also in a different state right now.

Debugging at a remove is always hard; it would be easier if your friend were to post here herself.
That being said, and please keep in mind that I am not a Mac guy by any stretch, what jumps out immediately to me is that our MacOS binaries are compiled for 10.11.5, I am unsure whether they can run on 10.9.5.

Ugh, yes, I got this exact error while setting up a friend's install on OS X recently. We ended up fixing it. Let me see if I can remember how...

Make sure you have OpenAL installed, then try using one of the builds from here?

That did it! Thank you! :D She's managed to kill herself by flying into an exploding TAC 1, but otherwise, things are fine.
