A-1 SUPAR was first said by Plasma on the VBB, pertaining to a conversation someone over posting a message that could have been sent via email, in to which Plasma replied to said topic with:

e-mail is A-1 SUPAR!!
VBB survivor: June 1999 until the end. Only banned 2 or 3 times (I think)

i don't know, but it's been around for a while, i don't think anyone at the VBB discovered it.  if they did i never knew about it.
