I installed FSO two days ago on my HotU FS2 (with all the missing files). It worked great. I was having no problems whatsoever; it ran nice and smooth. That same day, I downloaded a Windows update and applied it when I shut down my PC.

The next day, there was chaos. The first thing I noticed when trying to play FSO again was that the hud was flickering when large ships came into view. Then I noticed horrible texture flashes and framerate drops. Explosions and other effects appeared as large squares with random colors. After a few minutes, it would eventually freeze.

I run it on D3D, and it happens with or without all features enabled. I made no changes in settings from the day I installed it to the next time I played it. The only thing an accusing finger can be pointed at would be the Windows update.

I've tried various troubleshooting measures, eventually ending up with a FSO that would not even start up, giving me:

Error initializing graphics
Failed to get init mode

I've already tried reinstalling it, but to no avail. Please help me. This is an incredible game and I would like for it to work correctly. :(

Funny that you would mention having trouble with it. Someone in one of my classes this morning was saying that it made his computer unable to play perfectly good ISO's.

As for your problem, try uninstalling that update.
"The reason for this is that the original Fortran got so convoluted and extensive (10's of millions of lines of code) that no-one can actually figure out how it works, there's a massive project going on to decode the original Fortran and write a more modern system, but until then, the UK communication network is actually relying heavily on 35 year old Fortran that nobody understands." - Flipside

I would like to, but it appears that there were multiple updates, and I'm not sure which ones I should remove . . . I'm afraid I might accidentally screw up Windows. Then I'll REALLY have a problem.

Okay, I've uninstalled a couple of the updates and have noticed a considerable increase in stability in FSO. No more crashes, just occasional flickering and some slight framerate drops.

There you go. Btw, do you have any idea what those deleted updates were supposed to do?

Hmmm... security updates shouldn't cause such an instability on any machine, they're supposed to fix things, not worsen them...

Since i never had any problems with any security update:

Is it possible you accidentally downloaded and installed an MS driver for your graphics card? If so, behold the source of your trouble.
MS drivers are generally outdated, especially on graphics cards and i wouldn't be surprised if they produced what you described above.

When you click to uninstall the updates, you are given a list of all the programs affected by it. FSO was at the top of the list. As for video drivers, yes, that's what I was thinking. I'll have to reinstall those Geforce drivers on my Battlefield: Vietnam CD until I can download the latest ones. ;)

I actually also downloaded several security updates yesterday but haven't noticed any problems at all. The Windows Update 5 applet has some "express setup" option; does this install hardware drivers? The drivers they have on there are frequently very outdated (e.g. the nvidia drivers that show up are almost 10 months old), so using that express thing might cause it.

I hate WU 5 though. It requires you to set the auto updating service to automatic work at all (manual doesn't work), which is something I always otherwise set to disabled. Very annoying.

Well, I'm on Windows 2000 if that helps. I'll try the latest nVIDIA drivers and see how that works.

What version of directx you have there?
Is the data directory generated?
