Niven was probably killed during the last mission of the first SOC loop. I don't remember if he was one of the pilots there.
Rusk was rescued before the Iceni exploded, and the other two guys where those that left the Iceni with Bosch IIRC.

I actually just dropped in whatever I could remember from FS2 into that list; the dead guys can be useful when creating missions that occur during the same timeframe as the FS2 campaign. (i.e. many of the multiplayer missions)

Forsythe was also probably still alive at the end.

The names from one of the debrief stages, it happens if you loss a certain number of transports and given orders to one of the fighter wings. Story is the Shivan's got his transport 'cos you ordered his escort wing to do other things.
