The guy was a suicidal cop with machine guns.   Most of the time storming the bus results in no hostages killed.  Having training + automatic weapons made the situation pretty much impossible to diffuse.  Don't take action and he kills them all or risk it and hope for the best.  In this case the best didn't come out too well.

The consensus from most ERT specialists in other countries interviewed is that they ****ed this one up badly.  Of course, the hostage that was shot trying to rush the hostage taker probably accelerated things.

I'm not sure where to begin this one...

First of all, it appears they were not prepared for such a situation.

Notice that:

They hammer holes in the windows and door (dropping the hammer in the process), and then they don't do anything.
One dood gives the other a shield, after the other leaves the dangerous area of the door/windshield, then moves there himself without it.
WTF with the blue light stick toss at about 2:00?
After they play around with kevlar, riot shields... some guy without a gun comes to action in a baseball cap to put a chain on the door frame.
Before the door opening op fails, notice a bunch of cops taking cover in front of a pickup truck that's about to violently jerk forward if the door doesn't hold.
And finally, after building all those holes in the bus, they take a single entrance- the back door. Seriously, if they were planning (OK, I'm overdoing it with the word "planning") to get in using the rear door, why did they wreck nearly every window, the front door, and why did they swarm that bus for a few minutes in the first place?

And finally- when one person is doing something, he's always alone in the dangerous area, everyone else is busy doing everything NOT to give any overwatch.

I'm not really comfortable with playing armchair TOC on this one.

God knows US SWAT butchers far simpler situations all the time. These situations are so tense and complex that I'm not sure I'd trust anyone to get the job done reliably with no casualties.

You know, I don't think they messed up that bad.  It was a crappy situation that had a huge chance of going pear shaped with so much as a feather brush.
