Some companies are willing to replace broken CD's, if CD's were not mishandled and broke in normal usage.

Bog standard DVD case, I guess.... tho the spindle was somewhat larger that other ones I 've got.  

Had the game for (about) 2 years old now, natch....probably sod all chance of getting a replacement.  Not thatr it really matters.... I just wanted to fly in a chopper for a wee bit :( *sniff*
