Somebody tried to edit the pirate filter, and instead broke it.   This meant all forums which had the filter active (namely, most of the public forums) were inaccessible.

I wish I had known about this earlier.  More than that, I wish the culprit had stuck around long enough be sure that his changes worked...

I don't think any of the admins are that malicious.  They may have wanted to change it slightly, or they may have wanted to modify it, but I doubt any of them wanted to break the forum.

And it's quite possible that it wasn't even one of the admins, that it was simply a bug.  None of the functionality was actually changed; the only thing that happened is a few asterisks were added.  This caused an error with the regexp parser.
